CEREAL-BASED MEALS DIETARY HISTORY

 

CONSENT

This study is being conducted by a group of research scientists from the Department of Food Science and Technology, of the Kwame Nkrumah University of Science and Technology (KNUST). There are findings from studies that suggest that the food we consume may expose us, often, to low levels of harmful substances such as mycotoxins. Mycotoxin contamination mainly occurs in cereal crops and hence cereal based foods. This questionnaire is being administered to better understand the cereal-based foods consumption patterns among the population.

The data collected from this survey is strictly for academic and research purposes, will not be linked to respondents in any way, and will be confidential. We will appreciate you consenting to participate in the study and taking a few minutes to answer questions about the foods you eat most often (frequently consume).
